Current evidence does not support a simple conclusion that equal quantities of aspartame and sugar are either equally safe or that one is universally safer than the other; they present different health considerations under different exposure levels.

Both aspartame and sugar have extensive scientific literature evaluating their health effects, but they are associated with different risks. Regulatory agencies worldwide have established acceptable daily intake (ADI) limits for aspartame and continue to conclude that exposure below those limits is acceptable for the general population, excluding individuals with phenylketonuria (PKU). Sugar is not subject to an ADI but is associated with increased risk of several chronic diseases when consumed in excess. Existing evidence does not support reducing the comparison to a single overall "safer" verdict.

Confidence — current state

The strongest evidence comes from regulatory risk assessments, toxicology studies, randomized clinical trials, and long-term epidemiological research. Aspartame's safety evaluations primarily assess toxicity and cancer risk at specified exposure levels, while sugar research focuses largely on metabolic disease risk from chronic dietary intake. Because the substances differ chemically, nutritionally, and in sweetness, equal-weight comparisons are not equivalent to equal-sweetness comparisons.
