However, these studies face important limitations, including confounding, exposure measurement uncertainty, and inability to establish causation. Large sibling-controlled analyses have not found increased risk after accounting for familial factors. Regulatory agencies and major medical organizations have not concluded that a causal relationship has been established. Litigation concerning these claims has proceeded independently of the scientific evidence and has included federal court rulings regarding the admissibility of expert testimony.

Confidence — current state

The strongest available evidence consists primarily of observational cohort and case-control studies, systematic reviews, and meta-analyses. These can identify statistical associations but are generally limited in their ability to determine causation because treatment is not randomly assigned and important confounding factors may remain. Sibling-controlled studies that better account for shared familial factors have not found increased autism or ADHD risk. Scientific reviews continue to evaluate whether observed associations reflect a causal effect, underlying maternal illness, genetic factors, indication for medication use, or other sources of bias.
